The survey is designed to be brief and should take about 5 minutes to complete. You can participate in the following ways:

● In-Person: The printed version is available from the ushers, in the church office, and at the Mid-week@CLP Wednesday night dinner.
The survey will remain open through Sunday, March 29. We encourage you to share your thoughts as soon as possible so we can begin evaluating the results during our next committee work session. If you would like to complete this form anonymously, please pick up a physical copy on Sunday during worship.
