This form is for groups outside of our church family. If you are part of CLP ministries, please use the "CLP MINISTRY Facilities Use Request Form."
In order to give your event the attention and support it deserves, we kindly ask that Facilities Request Forms be submitted at least 45 days before your event date. This allows us to coordinate resources, volunteers, and calendar scheduling effectively.
If your request falls within a shorter timeframe, please contact the church office, as exceptions may be made with approval.
If your event does not require support from Facilities or AVL (Audio/Visual/Lighting) staff, a church key may be checked out to a qualifying individual for limited, short-term access. To be eligible, the individual must present a valid Arizona driver’s license and provide local contact information. This is a temporary key checkout, and the key must be returned to the church office within three (3) business days following the event.
