This form is for active ministries within our church family. If you and your organization are not part of our church family, please use the "Facility Use Request Form."
In order to give your event the attention and support it deserves, we kindly ask that the CLP MINISTRIES Facilities Use Request Form be submitted at least 45 days before your event date. This allows us to coordinate resources, volunteers, and calendar scheduling effectively.
If your request falls within a shorter timeframe, please contact the church office, as exceptions may be made with approval.
For CLP ministry events, please indicate who will be the responsible person to open and close the facility. If a long-term key holder is part of your ministry or event team, that individual may be assigned this responsibility. If no long-term key holder is available, a short-term key checkout may be arranged through the church office. The designated individual must provide a valid Arizona driver’s license and local contact information. Keys are issued for short-term use only and must be returned within three (3) business days following the event.
